Kimes Nursing and Rehabilitation Center is Medicare and
Medicaid Certified. Private pay and private insurance is
also welcome.
We offer competitive semi-private and private room rates and
admissions are accepted seven days a week. |

Admission Requirements |

|
Admitted from Home/Community Setting
- History and Physical exam (within last 30 days)
- Physician's orders for admission to Kimes Nursing &
Rehab Center
- Health insurance cards (Medicare, Medicaid, Private
Insurance, etc.)
- Social Security card

Durable Powers of Attorney for Healthcare, Guardianship,
Conservator, Health Care Surrogate (Not required)
- PASSAR screen
- Level of Care(Medicaid only)
Admitted from Hospital Setting
- Health insurance cards (Medicare, Medicaid, Private
Insurance, etc.)
- Social Security card

Durable Powers of Attorney for Healthcare, Guardianship,
Conservator, Health Care Surrogate (Not required)
- Hospital provides all other necessary information
A resident cannot be admitted without all necessary medical
forms. To assure a current History and Physical, an appointment
with the patient's attending physician may be required.
A resident must select a physician upon admission to the
facility. If the physician of resident's choice is not already
credentialed at Kimes Nursing & Rehab Center, credentialing
arrangements MUST be made with physician prior to admission.
For an admission to be covered in full or part by Medicare,
certain requirements must be met prior to admission.
Federal and Ohio law and rules require that the resident's
attending physician see the resident within 48 hours of
admission, every 30 days for the first 90 days and a minimum of
every 30 to 60 days thereafter. |
